You're in. Your tracker is on its way.
Your free 7-page Back & Sciatica 30-Day Tracker should arrive in your inbox within the next few minutes. Open it, print it, and keep it somewhere you'll actually see it.
Three simple steps to get started
No complicated system. Just a page, a pen, and about two minutes a day.
Check your email
Look for a message from Lynne Wimmer lynne@lynnewimmerdvm.com with your PDF link. Don't see it? Peek in your spam, promotions, or updates folder and mark it as safe.
Keep it within reach
Print all seven pages and set them by your bed, your coffee cup, or wherever you pause each day. Start with "My Starting Point" on page one.
Share what you notice
Take your tracker to your next appointment. Instead of "I think it's been worse?" you'll have dates, details, and better questions to ask.
Seven pages. Thirty days.
One clearer story.
Pain rarely follows a straight line. It shifts with sitting, walking, sleep, appointments, and medications. These pages help you catch those shifts before the details fade.
How to Fill It In & My Starting Point
Simple shorthand for pain, location, tingling, and strength, plus a place to record where things stand today.
Daily Tracking, Days 1–30
One row a day for pain, sensations, sitting, walking, sleep, and the medications, therapy, and care you used.
Symptom Map & What Affects It
Mark where sensations travel, left or right, and notice which everyday activities feel easier, the same, or harder.
Why write it down at all?
Was it the new chair? The physical therapy visit on Tuesday? The ibuprofen, the walk, or the night you finally slept? Memory blurs those days together, and by your next appointment the details have faded. A record lets you look back and see what came before a change, not just how you feel right now.
Fill in one row at roughly the same time each day. Short notes work. Patterns appear later.
Contact a healthcare professional promptly for new or increasing leg pain or numbness. Seek emergency care immediately for new or worsening leg weakness or foot drop, numbness around the groin or bottom, new difficulty urinating, inability to urinate, new loss of bladder or bowel control, or severe or worsening symptoms in both legs.
This tracker supports observation and conversation. It does not diagnose a condition or replace individualized medical care.
